It's required to create a PHP-MYSQL appointments reservation system to be destributed on the internet . the system should have a :
1)simple visitor like the attached screan shot that help visitors to:
a)login to the system (if login is required by admin)
b)book , confirm and manage thier appointments easily

2) an admin part to adjust the application settings (weekends , time intervals, if login is required, if admin-approval is essential before setting an appointment of the , front end theme .....extra) all settings will be sent along with an ER diagram of the Database .
This task include both graphic designing and development of the web application application , examples and screan shots will be sent
