Need this simple PHP form coded by anyone (only the material in popup on drafts), please look at link: http://www.lumbini.se/gf/

This form will generates an e-mail and a "thank you page". The information from the form should be presented and put together in the same way as the draft shows. 

Form buttons is dafault,  genererated e-mail contains similar text as "thank you" page.

On the date chooser, Sunday should not be able to choose becuase of buisiness is closed that day.

The web is hosted on a Unix server.

I´ll give you the FTP login and PSD draft by e-mail.
