I am looking for someone to quickly (within 48 hours) develop a simple php script that will accomplish the following task.  

Allow user to upload a .csv file containing approximately 300-20000 10 digit US phone numbers, one per line.  The script then needs to perform a reverse lookup on these numbers with a public lookup tool and return the results formated and plotted into a google earth compatible map layer file.

All code must be documented and written in english.
