Looking to have a Google Map embedded in a web page of our webiste

I would like to have an admin screen that contains these fields. When I click submit it writes to the XML file that the Google Map will pick up on.
This will place a pointer on Google Map. When the customer places their mouse over the pointer a popup box displays the information below.
If the field is blank (ex. no Facebook page) that field will not display.
Website, Facebook Page, Twitter, YouTube, Blog, all will be URL's that should be live where the customer can click on and open a new window (_blank)
