I need a JS script and PHP application that I can implement on my client's existing web site www.celtic-cards.com

You can choose, design and preview a memorial card including uploading your own photo, which you can then crop, and your own verse typed in to a form.

www.celtic-cards.com is a content management site but I can link to their own server at www.celtic.cx for the design-you-own-card page.
The implementation would have to be pretty simple as I am used to designing and hosting static web sites and whilst the celtic.cx server runs phpMyAdmin - 2.11.9.5 I haven't a clue about dynamic html.

Celtic Cards currently has 56 stock pictorial designs and 26 text verses to choose from but would like to be able to add more on an ad-hoc basis. If you google "memorial cards" or "memoriam cards" they are right up there in SE positioning behind JJ Lalors the market leader and they would like the site to have the functionality of the example given (who appear on the same results page as a paid or sponsored link).

Celtic Cards are a very small company and I have done some freelancing. However as far as the product goes they have received testimonials from around the world and some customers have come to them after having been to one of the other larger companies and got the whole job done over again properly, as they feel there is a lack of personal attention in the competition.

I'm looking for someone with PHP and Javascript skills but only because these seem on my initial inspection to be what the competition uses. Basically whatever the example needs to work we will need also.
