I contracted the services of a JOOMLA developer to make a form, but the developer was unable to finish the job.

I need to create a user friendly form to replace this one:

This form should have validation; 
the fields should be proportional to the type of values entered; once the values are entered, the form should be emailed and also stored in the database
The form is in MySQL, joomla and PHP

A tested, user friendly and professional form must be delivered.
